Patients can request their medical records by coming by our office and filling out a request sheet. Please note that a valid photo ID is required when requesting records. We do not release records to siblings, grandparents or family members at any time and we do not release records to parents of patients over the age of 18 without proper authorization. If you fall into one of these categories, we must receive proof of legal guardianship or court-appointed power of attorney documentation before releasing any records.

In the case of a deceased patient, we require the same legal documents or we need verification that the requester’s name is listed on the death certificate. If you have any questions or need assistance with this process, please contact our office.
